Its a doubt that everybody has whether, a cap/crown is necessary or not, or if a tooth needs cap/crown then after how much time. But the answer to this question cannot be decided by your own. To get the right answer you should visit your doctor. And for this, dentist look for 2 things i.e
1)After RCT how much sound tooth structure is left.
2)second is that how much force is there on the tooth or pressure or weight is there on the tooth.
When RCT starts, the remaining dentin of the tooth seems to dry out which means it becomes weak. For example a dry tree which stands still , but slowly the branches start cracking, some parts of the tree breaks and falls apart and it wont be able to stand in front of strong winds. Like this during the procedure of root canal treatment the dentin of your tooth becomes dry and the supply of nutrition and blood stops.
If after RCT, less sound tooth structure is remaining and even if it can handle more filling, it can break after sometime. But if the size of the filling is small which means maximum tooth is good and strong then it can run for a longer period of time, this has been observed only in some cases.
But if your thinking that after RCT and cap nothing will effect the tooth, then you are wrong. It has been observed that the cavities inside the tooth are usually deep and it makes the tooth weak even after placing a cap on it. The tooth can’t withstand hard forces and eventually it breaks.
To place a cap, the dentist has to reduce the tooth structure from all the four sides making it small in size. This ultimately leads to weakening of the tooth. In this scenario before placing the cap, the tooth requires post and core treatment , so that tooth gains some strength.
Now the second most important thing is that the dentist has to calculate how much force is being applied on the tooth. In general the back teeth (molars) bears more pressure as compared to the front teeth (the incisors). That’s why the back teeth requires a cap which is necessary. But in some cases it is observed that opposing tooth in the molars are missing and if its missing then there is no pressure on it and it can work without a cap if only the filling in that tooth is small and has it has enough sound tooth structure.
One more important and common question that everybody has is, After how much time should the cap be placed after RCT? And the answer to this question is after RCT the dentists recommend that within a week the Cap should be placed so that your teeth don’t break.
If the dentist has recommended you for the cap/crown you must get it done and a patient cannot decide when it should be placed, whether its necessary or not. One should always listen to their dentist.
